Tap Water and Baby Formula: What Parents Need to Know
For powdered infant formula, the WHO recommends mixing with water that is still at least 70°C (158°F) — hot enough to kill bacteria in the powder itself, not just the water. This applies even in countries with perfectly safe tap water, because the powder, not the tap, is the usual contamination source.
Why 70°C: The Risk Is in the Powder
Powdered infant formula is not sterile. It can carry Cronobacter sakazakii and Salmonella — rare contaminants, but ones that cause severe bloodstream and brain infections in young infants, with Cronobacter meningitis carrying high mortality in newborns. This is why the WHO's preparation guidance centers on the powder: boil water, let it cool for no more than 30 minutes so it remains at 70°C or above, mix the formula, then cool the bottle quickly (under running water or in an ice bath) to feeding temperature and test it on your wrist.
At 70°C, the water pasteurizes the powder as it dissolves. Water that has cooled to hand-comfortable temperature before mixing kills nothing. The highest-risk groups are babies under two months, premature infants, and immunocompromised babies — for them, the 70°C rule matters most. Feed prepared formula within two hours (or refrigerate immediately and use within 24), and discard whatever the baby doesn't finish.
Boil the Water Even in Safe-Water Countries
Separately from the powder question, most national health agencies (including the NHS and Health Canada) advise boiling tap water for formula for babies under 6 to 12 months even where adults drink the tap freely. Infant immune systems and stomach acidity are immature, so the trace microbial levels acceptable in adult drinking water get less benefit of the doubt. Boiling once, then using the water at 70°C for mixing, satisfies both rules in a single kettle cycle.
In countries rated Caution or Not Safe on our safety map, tap water should not be used for formula at all without boiling — and many traveling parents reasonably prefer boiled bottled water there, since boiling addresses microbes but not chemical contaminants in a poor supply.
Nitrates: The Well Water Warning
If your home uses a private well, nitrates deserve specific attention. Nitrate contamination — typically from agricultural runoff or septic systems — is dangerous to infants under six months even at levels adults tolerate, because it interferes with oxygen transport in the blood and can cause methemoglobinemia, or “blue baby syndrome.” The US EPA limit is 10 mg/L nitrate-as-nitrogen for public supplies, but private wells are unregulated and untested unless you test them.
Critically, boiling makes nitrates worse, concentrating them as water evaporates. If you're on a well and formula-feeding, have the well tested for nitrates and coliform bacteria before the baby arrives, and use an alternative water source if nitrate results are elevated. Reverse osmosis and ion exchange remove nitrates; boiling and carbon filters do not.
Fluoride: A Minor, Manageable Consideration
In areas with fluoridated tap water (around 0.7 mg/L in the US), exclusively formula-fed babies get more fluoride than breastfed babies. The only documented consequence is a slightly higher chance of mild dental fluorosis — faint white streaks on developing teeth that are cosmetic, not harmful. The American Dental Association considers fluoridated tap water acceptable for formula, while noting that parents who want to minimize fluorosis risk can alternate with low-fluoride bottled water (labeled purified, demineralized, deionized, or distilled).
This is a preference decision, not a safety one. No mainstream body advises avoiding fluoridated tap water for formula, and the cavity-prevention benefits of fluoride are real once teeth erupt.
Choosing Bottled Water for Formula
When bottled water is the right call — while traveling, or on a problematic well — not every bottle is appropriate. Some mineral waters carry sodium and sulfate levels fine for adults but poorly suited to infant kidneys. Look for water with sodium (Na) under 200 mg/L — and ideally much lower, under 20 mg/L — and sulfate (SO4) under 250 mg/L; European guidance for infant-suitable water is commonly stricter still. Low-mineral purified water is the simple safe choice.
Remember that bottled water is not sterile either: for babies under the age thresholds above, bottled water should also be boiled before mixing powdered formula. The exceptions to all of this are ready-to-feed liquid formula, which is sterile and needs no water at all — the recommended option for premature and newborn babies when affordable — and, of course, breastfeeding.
Traveling with a Formula-Fed Baby
Travel compresses all of these decisions into unfamiliar kitchens. The workable routine: a travel kettle (or the hotel kettle — run a cleaning boil first, as covered in our hotel water guide), low-mineral bottled water in Caution and Not Safe countries, and pre-measured powder in a dispenser. Ready-to-feed cartons, while bulky and pricey, remove every variable and are worth their suitcase space for the first travel days in destinations like Mexico or India.
Wash and sterilize bottles with safe water too — boiling water poured over washed bottles works anywhere. A bottle carefully filled with purified water and then rinsed under a contaminated tap undoes the whole exercise.
